The New York Certificate of Change (For Use By Agent) (Domestic Corporation) is a form used by domestic corporations in the state of New York to make changes to their corporate structure. These changes may include the name of the corporation, the address of the corporation, the names and addresses of the directors, the registered agent, the purpose of the corporation, the capital structure, and the amount of shares authorized. There are three types of New York Certificate of Change (For Use By Agent) (Domestic Corporation): Amendment, Merger, and Consolidation. All three forms require the signature of the registered agent of the corporation, and the filing fee.
