The Nebraska Tobacco Manufacturer Certification Form is a document used by tobacco manufacturers to certify that they are compliant with the Nebraska Tobacco Product Manufacturer Act. The form requires the manufacturer to provide information such as the company name, address, contact information, products manufactured, and other information related to their tobacco product manufacturing. There are two types of Nebraska Tobacco Manufacturer Certification Forms: one for cigarettes, and one for smokeless tobacco products. Both forms require the same information, but also include additional questions related to the specific product. Upon completion and submission of the form, the manufacturer will receive a certificate of compliance that is valid for a period of two years.
