Texas Licensee Name/Address Change Request Form is a document used to notify the Texas Alcoholic Beverage Commission (TAB) of any changes in the licensee name and/or address of an existing business. This form is used to ensure that the TAB is aware of all changes to the licensee information, in order to keep accurate records and ensure compliance with all applicable laws and regulations. There are two types of Texas Licensee Name/Address Change Request Forms: one for alcoholic beverage permit holders and one for mixed beverage permit holders. Both forms require the licensee to provide their current name, address, and telephone number, as well as the requested changes. The forms must be signed and dated by the licensee, and a copy of the form must be submitted to the TAB for review.
