Maryland Consents To Change of Name (Consent of Parent Guardian Or Custodian) is a legal document used to change the name of a minor. This document must be signed by the parent, guardian, or custodian of the minor in order for the minor to legally change their name. There are two types of Maryland Consents To Change of Name (Consent of Parent Guardian Or Custodian): a Consent of Parent and a Consent of Guardian or Custodian. The Consent of Parent allows the parent to consent to the change of name of their minor child, and the Consent of Guardian or Custodian allows someone other than the parent to consent to the change of name of the minor. Both documents must be filled out and signed in the presence of a notary public. The documents must also be filed with the Clerk of the Circuit Court in the county where the minor resides.
