The Indiana Report of Application and/or Order Authorizing Interception of Communications (State/County/Local) is a form used by law enforcement agencies in Indiana to obtain a court order authorizing the interception of communications. The form must be completed by the law enforcement agency requesting the interception and submitted to the court for approval. The form includes information such as the identity of the law enforcement agency making the request, the purpose of the proposed interception, the type of communication to be intercepted, the proposed duration of the interception, and the specific location of the interception. It also must include a detailed explanation of the facts and circumstances that support the request, including the facts and circumstances that lead to the belief that the interception of communications is necessary. There are two types of Indiana Report of Application and/or Order Authorizing Interception of Communications (State/County/Local): one for state level agencies and one for county or local level agencies. The state level form must be approved by the Indiana Supreme Court, while the county or local level form must be approved by the circuit court of the county in which the interception is to take place.
