Washington Nurse Case Management Initial Care Management Plan is a program developed by the State of Washington to provide coordinated access to health care services for patients. It enables health care providers to develop an individualized care plan that focuses on the patient’s specific needs and goals. The plan is designed to support the patient’s ability to manage their health and prevent the development of acute or chronic illnesses. There are two types of Washington Nurse Case Management Initial Care Management Plans: 1) Comprehensive Care Plans and 2) Transition Care Plans. Comprehensive Care Plans are designed to meet the long-term health care needs of patients and focus on preventive care, while Transition Care Plans are designed to meet the short-term needs of patients and focus on acute care.
