Washington CCP Initial Response and Assessment Form I is a form used by the Washington State Department of Children, Youth, and Families (DCF) to document a child or youth's initial response to a particular situation and assess their needs. The form is used by DCF staff to record a child or youth's initial response to a crisis situation, assess their risk level, and determine if they require further assessment or intervention. There are two types of Washington CCP Initial Response and Assessment Form I: one for children and one for youth. Each form contains information about the child or youth's current crisis, including any safety issues, risk or protective factors, and age-appropriate interventions or services needed. The form also includes questions about the child or youth's family, school, community, and cultural background to help DCF staff make an appropriate assessment.
