The Virginia Amendment Cover Sheet to Accompany Any Amendment Filed by the Debtor to the Original Lists, Statements and/or Schedules is a form that must be filled out and submitted when a debtor amends any information on the bankruptcy petition. The form must be filled out by the debtor and includes sections for the debtor's name, case number, date of filing, and a statement that the debtor has reviewed the amended forms and believes the information is correct. The form must also be signed and dated by the debtor. There are two types of Virginia Amendment Cover Sheet to Accompany Any Amendment Filed by the Debtor to the Original Lists, Statements and/or Schedules. The first is the "Amendment Cover Sheet," which should be used when the debtor is amending the original petition. The second is the "Amendment Cover Sheet (Change of Address)" which should be used when the debtor is changing the address on the original petition.
