It is a sworn statement that is prepared by a party in a legal dispute, in this case, a moving, and is used to request permission to appear before a judge or court to present their case. The affidavit must include the moving's name, the case number, the court in which the case is being heard, the moving's legal interest in the case, the reasons why the moving wishes to appear, and finally, the moving's signature.
