Pennsylvania Eligible Organization Games of Chance Application (180.5 KiB) is a form that must be completed and submitted by eligible organizations in the state of Pennsylvania who wish to use games of chance to raise money for their organization. This form is necessary for organizations to be allowed to legally conduct games of chance such as bingo, raffles, and chance drawings. The application must be completed in full and submitted to the Pennsylvania Department of Revenue, along with any additional documents that may be required. The application includes sections for general information about the organization, the type of game of chance the organization is looking to conduct, the type of prize or cash payout associated with the game, and the dates that the game will take place. Once the application is approved by the Department of Revenue, the organization will be granted a Games of Chance License and will be able to conduct the game of chance as described in the application.
