The Pennsylvania Language Access Complaint Form is a document meant to help individuals who speak a language other than English to file a complaint about their lack of access to language services in the Commonwealth of Pennsylvania. This form is meant to allow individuals to submit complaints about the lack of or inadequate provision of language services such as translations, interpretation, or other language assistance that would help them better access public services or programs. The form also allows individuals to report cases of discrimination or mistreatment based on the language they speak. The Pennsylvania Language Access Complaint Form is available in two versions: English and Spanish. The form is free to use and is available online for anyone to download and fill out. The form must be completed in its entirety and submitted to the Pennsylvania Office of Open Records for review. Once submitted, the complaint will be reviewed by the Pennsylvania Office of Open Records and a response will be provided.
