The New York M-4 Supplemental Salesperson Statement is a document required by the New York Department of Taxation and Finance (DT) for businesses that employ salespersons in the state of New York. The form provides an estimate of the salesperson's New York State wages for the previous tax year. It must be completed and submitted to the DT with the employer's annual tax returns. The statement must include the salesperson's name, Social Security number, and total wages paid in the past year. The New York M-4 Supplemental Salesperson Statement comes in two forms: the Form M-4S for all salespersons and the Form M-4S-R for salespersons who are residents of New York State.
