The Maryland Request for Accommodation by Persons with Disabilities (RAPID) is a form used by individuals with disabilities to request modifications or accommodations in order to access services, activities, or programs. It is a tool used by state and local government agencies, educational institutions, and other public and private entities to ensure that individuals with disabilities have equal access to their services. The RAPID form provides a method for individuals to request reasonable and necessary accommodations that are specific to their disability and are necessary to ensure equal access. There are two types of RAPID forms: a Request for Accommodation form and a Request for Reasonable Accommodation form. The Request for Accommodation form is used when an individual needs a change in the physical environment or equipment in order to access a service, activity, or program. The Request for Reasonable Accommodation form is used when an individual needs a change in a policy, procedure, or rule in order to access a service, activity, or program.
