Maryland Petition for to Change of Name (Minor) is a legal document that allows a minor (under 18 years of age) to change their name in the state of Maryland. There are two types of Maryland Petition for to Change of Name (Minor): (1) Petition to Change Name of an Adult and (2) Petition to Change Name of a Minor. The Petition to Change Name of a Minor must be filed with the Circuit Court in the county where the minor resides. The Petition must include the current name of the minor, the proposed name of the minor, the reason for the name change, and the name and address of the minor’s parent or guardian. Once the Petition is filed, the Court will review the Petition and make an order allowing or denying the name change. The order must be signed by the judge and must include the new name of the minor. The Petition must be served upon the minor’s parents or guardians and other interested parties before the Petition is heard in court.
