The Illinois Application and Order for Transfer of Will is a form used by the Illinois Probate Court to transfer a will from the testator (the person who created the will) to the Probate Court for safekeeping. The form is most commonly used when the testator passes away. There are two types of Illinois Application and Order for Transfer of Will. The first is a Short Form, which is for cases in which the testator was not a resident of Illinois at the time of death. The Short Form must be filled out and signed by the executor or administrator of the estate, and must be accompanied by a certified copy of the will and proof of death. The second type is a Long Form, which is used for cases in which the testator was a resident of Illinois at the time of death. The Long Form must be completed and signed by the executor or administrator of the estate, as well as by the testator's attorney. It must also be accompanied by the original will, a certified copy of the will, and proof of death.
