Arizona Voluntary Acknowledgement of Paternity (AZ VAP) is a legal form available to unmarried parents in Arizona to establish the legal father of a child. The AZ VAP is a sworn statement, signed by both parents, in which they acknowledge their parental relationship to the child. The acknowledgement is then filed with the Arizona Department of Health Services, Vital Records Section. This legal document provides the father with the same rights and responsibilities as a father whose paternity has been established through a court order. There are two types of AZ VAP: the original AZ VAP and the AZ VAP Supplemental. The original AZ VAP is used when both parents are present to sign the form. The AZ VAP Supplemental is used if one parent is unable to sign the form, such as when one parent is unavailable or deceased.
