There are two types of Arizona Declaration of Petition Preparer: (1) an individual declaration, which is signed by a single individual; and (2) a business declaration, which is signed by two or more people who are in partnership or employed by the same legal entity. Both declarations require the petition preparer to provide proof of professional qualifications, such as a law degree or an attorney’s license. The declaration also requires the petition preparer to agree to abide by the Arizona Rules of Professional Conduct.
