Wisconsin Articles of Amendment — Cooperative is a legal document used to record the changes made to a Wisconsin Cooperative Association. This document must be filed with the Office of the Commissioner of Insurance and must be signed by the cooperative’s president, secretary, and/or treasurer. There are two main types of Wisconsin Articles of Amendment — Cooperative: 1) Articles of Amendment that amend the cooperative’s name; and 2) Articles of Amendment that make amendments to the articles of organization. The Articles of Amendment must include the name of the cooperative, its purpose, the names and addresses of its incorporates, the date of filing, and the specific amendments made to the articles of organization.
