Wisconsin Affidavit of Additional Property and/or Creditor for Summary Assignment (Formal Administration) is a form used by Wisconsin probate courts to provide additional information on the estate of a deceased individual. The form is used to declare any additional property or creditors of the estate that were not previously identified. There are two types of affidavits that can be used in this situation: an Affidavit of Additional Property and an Affidavit of Additional Creditor. The Affidavit of Additional Property is used to declare any assets not previously listed in the Summary Assignment, such as real estate, stocks, bonds, bank accounts, and other personal property. The Affidavit of Additional Creditor is used to declare any creditors not previously listed in the Summary Assignment, such as credit card companies, loan companies, or other businesses that the decedent may have owed money to. The Wisconsin Affidavit of Additional Property and/or Creditor for Summary Assignment (Formal Administration) must be completed and filed with the probate court in order to be considered valid.
