Washington Functional Recovery Interventions (RFI) Tracking Sheet is a tool used by mental health professionals in the state of Washington to track progress in the recovery process. It is a form that is used to record information such as mental health diagnosis, treatment goals, and service provided to individuals. The RFI Tracking Sheet consists of several sections including: client information, treatment goals, interventions, outcomes, and follow-up/monitoring. It is used to assess progress in treatment, identify areas of improvement, and identify any gaps in service. There are two types of Washington Functional Recovery Interventions (RFI) Tracking Sheets: the Client Tracking Sheet and the Program Tracking Sheet. The Client Tracking Sheet is used to track progress on an individual basis, while the Program Tracking Sheet is used to track progress at a programmatic level.
