Washington Functional Recovery Interventions (RFI) Tracking Sheet is a tool used to monitor and track the progress of individuals who are receiving functional recovery interventions (RFID) in Washington state. It is a standardized form used by providers, case managers, and other stakeholders to document and monitor the outcomes of RFID. The sheet includes the name of the participant, the intervention, the date of intervention, the goals of the intervention, the progress of goals, and the overall conclusion of the intervention. It also includes a space to document any additional comments related to the intervention. There are two types of Washington Functional Recovery Interventions (RFI) Tracking Sheet: the Standard Sheet and the Extended Sheet. The Standard Sheet includes the basic information about the intervention, while the Extended Sheet includes more detailed information about the outcomes. Both sheets are designed to help providers and stakeholders to track the progress of individuals receiving RFID in Washington state.
