The Intention to Pursue Complaint for RFA is used when the department has decided to pursue a complaint. This form includes the name of the complainant, the name and contact information of the respondent, the legal issue or issue of concern, the facts of the case, and the desired resolution. The Intention to Withdraw Complaint for RFA is used when the department has decided to withdraw a complaint. This form includes the name of the complainant, the name and contact information of the respondent, the legal issue or issue of concern, the facts of the case, and the reason the complaint is being withdrawn. Both forms must be completed and signed by a representative of the Vermont Department of Labor in order to be considered valid.
