Vermont Certificate of Service — Small Claims is a document issued by the Vermont Superior Court in order to provide proof of service of certain documents in a small claims action. There are two types of Vermont Certificate of Service — Small Claims: Type I and Type II. Type I is used to proving service by mail, and Type II is used to prove service by other methods such as personal delivery or publication. The certificate must be completed and signed by the person making service and must include the name and address of the person being served, the date and method of service, and the name and address of the person making service. It must then be filed with the court.
