The Vermont Affidavit in Support of a New or Delayed Birth Certificate is an affidavit that is completed by an individual when applying for a new or delayed birth certificate in the state of Vermont. The affidavit is used to provide evidence or proof of an individual’s birth in the state of Vermont, if the person’s birth was not registered with the state at the time of birth. The affidavit must be signed by two affronts whose knowledge of the birth was made at or near the time of birth, and who have personal knowledge of the facts of the birth. There are two types of Vermont Affidavit in Support of a New or Delayed Birth Certificate: 1) the Affidavit in Support of a New Birth Certificate and 2) the Affidavit in Support of a Delayed Birth Certificate. The Affidavit in Support of a New Birth Certificate is used to apply for a new birth certificate when one was not issued at the time of birth due to a birth occurring at home or other circumstances. The Affidavit in Support of a Delayed Birth Certificate is used to apply for a delayed birth certificate when an individual was born in Vermont but was unable to obtain a birth certificate at the time due to extenuating circumstances.
