Vermont Proof of Claim, Supplement 1 is a form that is used by creditors to claim a debt from a debtor in the State of Vermont. It is submitted to the court in order to allow a creditor to receive payment for a debt that they are owed. This form must be completed and filed with the court prior to the debtor's discharge date in order to be considered valid. The form must include the name of the creditor, the amount of the debt, the date of the debt, and the debtor's name and address. There are two types of Vermont Proof of Claim, Supplement 1: one for secured creditors and one for unsecured creditors. The secured creditor’s form requires information about the secured property, while the unsecured creditor’s form does not. In both cases, a signature from the creditor is required in order for the form to be valid.
