Vermont Proof of Claim, Supplement 2 is a form used by creditors in the state of Vermont to make a claim against a debtor's estate in bankruptcy proceedings. The form is provided by the US Bankruptcy Court for the District of Vermont and must be filed with the Bankruptcy Court along with a copy of the applicable Vermont state statutes. The form is used to provide information and documentation to the court, including the amount of the debt, facts supporting the claim, and proof that the debtor was served with notice of the bankruptcy petition.
