Texas CCP Figure 1 — Required Disclosure Statement For All Consumer Choice Health Benefit Plans is a document which is required to be provided by insurers to consumers for all consumer choice health benefit plans in the state of Texas. This document must be provided to the consumer in order to provide them with the necessary information they need to make an informed decision when selecting a health insurance plan. The document includes information such as plan name, plan type, coverage limits, co-payment amounts, coinsurance levels, and other important details. There are two types of Texas CCP Figure 1 — Required Disclosure Statement For All Consumer Choice Health Benefit Plans: the Individual and the Group version. The Individual version is for individuals who are purchasing health insurance on their own, while the Group version is for employers who are providing health insurance to their employees.
