The Pennsylvania Application for Involuntary Emergency Examination & Treatment — Spanish (Form PA-50SP) is a legal document used to initiate involuntary mental health treatment for a person in Pennsylvania who is believed to be in need of immediate evaluation and possible emergency treatment. This form is typically completed by a physician, mental health professional, law enforcement officer, or any other person authorized to sign it. The form is then submitted to the court for approval. Once approved, the court will appoint an examiner to evaluate the person and make a recommendation for treatment. There are two types of Pennsylvania Application for Involuntary Emergency Examination & Treatment — Spanish forms: Form PA-50SP-1 and Form PA-50SP-2. Form PA-50SP-1 is used to request an examination and/or emergency evaluation without hospitalization. Form PA-50SP-2 is used to request an examination and/or emergency evaluation with hospitalization.
