Cover Sheet

The Pennsylvania Cover Sheet is a form used to provide information about an insurance claim to the Pennsylvania Department of Insurance. It is completed and submitted with a claim form. There are two types of Pennsylvania Cover Sheets: The Pennsylvania Workers' Compensation Cover Sheet and the Pennsylvania Automobile Insurance Cover Sheet. The Workers' Compensation Cover Sheet is used to report claims for injured workers that are covered under Pennsylvania Workers' Compensation Law. The Automobile Insurance Cover Sheet is used to report claims for automobile damages that are covered under Pennsylvania Automobile Insurance Law. Both of these forms must be completed and submitted with the respective claim form to ensure that the claim is processed in a timely manner.
