Oklahoma Attachment D: Relocation Notification of Children is a document used to notify the Oklahoma Department of Human Services (DHS) when a custodial parent wishes to relocate with their children to another state. It is used to inform the DHS of the custodial parent’s intention to relocate and provide information on the new location, including the address and contact information. The document must also include contact information for the noncustodial parent and, if applicable, the attorney representing either parent in the custody case. There are two types of Oklahoma Attachment D: Notification of Relocation of Children: 1) Notification of Relocation of Children and 2) Notification of Relocation of Children with Minor Children. Both forms must be completed and filed with the DHS before the relocation can take place.
