New York Animal Bites / Rabies Report Form is a form used by medical providers, including veterinarians and laboratories, to report animal bites to the New York State Department of Health. The form is used to report all animal bites, including those from domesticated and wild animals, as well as those from animals suspected of having rabies. The form is used to collect information about the animal bite, such as the date, location, and type of animal involved, as well as the name, address, and contact information for the victim. The form is then used to help the Department of Health monitor and track potential rabies exposure. There are two types of New York Animal Bites / Rabies Report Form: the Animal Bite Report Form and the Rabies Suspect Animal Bite Report Form.
