The New York Title VI Non-Discrimination Complaint Form is a document used to file a complaint of discrimination based on race, color, or national origin in New York State programs or activities that receive federal financial assistance. The form is available in both English and Spanish and can be used to report any discrimination related to a person's race, color, or national origin in areas such as: employment, education, healthcare, housing, transportation, or access to public accommodations. There are three types of New York Title VI Non-Discrimination Complaint Form: Individual Form, Agency/Organization Form, and Multiple Complainant Form. The Individual Form is used to file a complaint from an individual who believes they have been discriminated against. The Agency/Organization Form is used to file a complaint on behalf of an agency or organization that believes it has been discriminated against. The Multiple Complainant Form is used to file a complaint from two or more people who believe they have been discriminated against.
