The New York Certification of Services Rendered is a form issued by the New York State Department of Taxation and Finance to certify the provision of services rendered to a customer. It is an official document that serves as proof that services have been performed and fees paid to the service provider. It is used to prove that services have been rendered and fees have been paid to the service provider for tax purposes. There are two types of New York Certification of Services Rendered: a Long Form and a Short Form. The Long Form is used for services that cost more than $5000, while the Short Form is used for services that cost less than $5000. Both forms require the customer to provide the following information: their name, address, and taxpayer identification number; the provider's name, address, and taxpayer identification number; the date and nature of the services rendered; the amount of the fee paid; and the provider's signature. The completed form must then be submitted to the Department of Taxation and Finance in order to receive a Certificate of Services Rendered.
