This notice is used when an attorney or a party is filing a case in the Supreme Court of New York, Appellate Division. There are two types of New York Notice of Hard Copy Submission — E-Filed Case: Notice of Hard Copy Submission of Index Number Form and Notice of Hard Copy Submission of E-Filed Case. The Notice of Hard Copy Submission of Index Number Form is used when a person is filing a case with an index number, whereas the Notice of Hard Copy Submission of E-Filed Case is used when a person is filing a case without an index number. This notice must be filed within 14 days of the electronic filing. It must include the e-filed case number, the attorney's name and contact information, and the documents that were e-filed.
