New York Revocation of Authorization for Electronic Filing — Single Attorney is a form used to revoke the authorization of an attorney to file legal documents electronically with the New York Unified Court System. This form must be completed and signed by the attorney in order to revoke the previously authorized Electronic Filing Authorization. There are two types of New York Revocation of Authorization for Electronic Filing — Single Attorney forms: one for attorneys registered with the New York Unified Court System e-Filing System, and one for attorneys not registered with the e-Filing System. Both forms require the attorney’s name, address, and contact information, as well as signature and date.
