The New York Claimant's Record of Independent Job Search Efforts is a document used by the New York State Department of Labor to keep track of a claimant's job search activities. This document is used to determine eligibility for unemployment insurance benefits. It records the names of employers contacted, dates of contact, the type of contact, and the results of the contact. It also includes other job search activities such as attending job fairs, job training, and use of online job search tools. There are two types of New York Claimant's Record of Independent Job Search Efforts: the Online Job Search Record and the Paper Job Search Record. The Online Job Search Record is a digital form completed on the New York State Department of Labor website. The Paper Job Search Record is a paper form printed from the NYS DOL website and completed by the claimant.
