The New York Claimants Record of Job Search Efforts Contacts is a document provided by the New York State Department of Labor (NYS DOL) that is used by unemployed individuals to show proof of their job search efforts. This document is a record of the contacts an unemployed individual has made with potential employers as part of their job search. It includes the names, addresses, and telephone numbers of all employers contacted, as well as the date of the contact, the method of contact (e.g. telephone, mail, email, in person, etc.), and a brief description of the position sought. There are two types of New York Claimants Record of Job Search Efforts Contacts: Personal Contacts and Internet Contacts. Personal Contacts are contacts made with employers in person or by telephone, mail, or email. Internet Contacts are contacts made through online job search resources, such as job boards, social networking sites, and other web-based job search tools.
