The New York Personal Information Form (Child Support or Paternity) is a form used for providing personal information required for establishing paternity or to obtain child support. It is used by the Child Support Enforcement Unit of the New York State Department of Taxation and Finance. The form includes information regarding the parent, such as name, address, Social Security number, and contact information. It also includes information regarding the child, such as name, date of birth, and Social Security number. There are two types of New York Personal Information Forms (Child Support or Paternity): the Paternity Acknowledgement Form and the Child Support Enforcement Form. The Paternity Acknowledgement Form is used by the father to acknowledge paternity and the Child Support Enforcement Form is used to initiate a child support order.
