The New York Child Support Agency Confidential Information Form is a document used to protect the confidential information of the child and the noncustodial parent. It is used to ensure that sensitive information related to the child’s identity, financial information, and personal information is kept confidential. The form must be filled out by both the custodial and noncustodial parent and submitted to the New York Child Support Agency in order to be processed. There are two types of New York Child Support Agency Confidential Information Forms: one for establishing paternity and one for setting up a child support order. Both forms require the noncustodial parent to provide their name, address, Social Security Number, and other personal information. The custodial parent must also provide similar information as well as information related to the child such as their name, date of birth, and address.
