The New Hampshire Second Injury Fund Affidavit of Employer Knowledge is a document that is used to verify an employer's knowledge of a workers' compensation claim that an employee has filed for a preexisting injury. It is a legally binding document that provides the Second Injury Fund with evidence that the employer was aware of the employee's preexisting condition prior to the work-related injury. This document is required for any claim to be approved by the Second Injury Fund. There are two types of New Hampshire Second Injury Fund Affidavit of Employer Knowledge: 1) an Employer Affidavit and 2) a Doctor Affidavit. The Employer Affidavit is completed by the employer, attesting to their knowledge of the employee's preexisting condition. The Doctor Affidavit is completed by a doctor, attesting to the employee's preexisting condition and their opinion of how the preexisting condition contributed to the current work-related injury.
