The New York Certificate of Termination of Existence (Of Foreign Corporation) Cover Sheet is a document that must be submitted to the New York Department of State (NYS DOS) when filing a Certificate of Termination of Existence (also known as an Articles of Dissolution, Certificate of Dissolution, or Certificate of Cancellation) for a foreign corporation. The cover sheet must be filed in addition to the Certificate of Termination of Existence form and any required documents. The cover sheet includes information such as the foreign corporation's name and state of incorporation, the name and address of the foreign corporation's registered agent in New York, the name and address of the foreign corporation's representative filing the certificate, and the name and address of the foreign corporation's principal office.
