This document is used when the court has granted a petition for the appointment of a guardian and/or conservator, and the appointed individual has accepted the responsibility. The document contains the name and address of the appointed guardian and/or conservator, a description of the authority granted to the guardian and/or conservator, and the signature of the appointed guardian and/or conservator.
