The Montana Recap Sheet legal-size (also letter-size) is a document used by employers in the state of Montana to record and report their payroll records. It is a double-sided 8-1/2” x 14” form with two columns on each side. The left column is for the employer's information, including the employer's name, address, and unemployment insurance account number. The right column is for employee information, including employee name, social security number, wages earned, and taxes withheld. The bottom of the form includes the employer's signature and wage information summary. The Montana Recap Sheet letter-size is a downsized version of the legal-size form, measuring 8-1/2” x 11”. The information required is the same, but the form is smaller. Both forms are used to report employee wages to the Montana Department of Labor & Industry.
