The Montana Recap Sheet legal-size (also letter-size) is an official form used by the state of Montana to report specific information related to unemployment insurance claims. It includes the claimant’s name, Social Security number, the total amount reported in wages for the claim period, the total amount of benefits paid, the total amount of benefits owing, and the amount of benefits due the claimant. It also provides a summary of the claimant’s wage and benefit information for the current claim period. The Recap Sheet is used to assist the Montana Department of Labor and Industry in determining the claimant’s eligibility for unemployment insurance benefits. The Recap Sheet is available in both legal-size and letter-size formats.
