A Missouri Broker Dealer Affidavit is a legal document used by the Missouri Division of Finance when registering a broker-dealer or investment adviser. It is a sworn statement that certifies that the applicant has met the requirements of the state securities laws and regulations and is authorized to act as a broker-dealer or investment adviser in the state of Missouri. The Affidavit is sworn by an authorized representative of the broker-dealer or investment adviser, and must be signed and notarized. There are two types of Missouri Broker Dealer Affidavit forms: the Initial Registration Affidavit, which is used to register a broker-dealer or investment adviser for the first time, and the Renewal Affidavit, which is used to renew the registration of a broker-dealer or investment adviser.
