A Minnesota Conservatorship Account Confidential Information Form is a legal document used in Minnesota to protect the confidential information of a conservatorship account, which is a financial arrangement wherein a conservator is appointed to manage another person's financial affairs. The form is used to provide a list of confidential information regarding the conservatorship account, such as the names of the parties involved, the account numbers, the type of account, the amounts held in the account, and any other relevant information. There are two types of Minnesota Conservatorship Account Confidential Information Forms: one for an account opened prior to July 1, 2009, and one for an account opened on or after July 1, 2009.
