The Massachusetts Petition for Change of Name(s) (Form CJP-27) is a legal document used to change an individual’s name, or the names of multiple individuals, in the Commonwealth of Massachusetts. It is typically used when an individual would like to change their name for personal or professional reasons, or when two people are getting married and both want to take the same last name. There are two types of Massachusetts Petition for Change of Name(s) (Form CJP-27): Adult Name Change Petition (Form CJP-27A) and Minor Name Change Petition (Form CJP-27B). The Adult Name Change Petition must be completed and signed by the individual seeking the name change, and the Minor Name Change Petition must be completed and signed by the parent or guardian of the minor. Both forms must be notarized and submitted to the court.
