Louisiana Debt — Activation Form (PDF) is a form used by Louisiana Department of Business and Professional Regulation (Debt) to activate and register businesses operating in the state of Louisiana. It is a comprehensive document that must be completed to obtain a business license and to register with the Debt. It contains a variety of information, such as the business name, address, contact information, type of business, ownership details, and other business-related information. There are two types of Louisiana Debt — Activation Form (PDF): the Business License Activation Form and the Business Registration Form. The Business License Activation Form is used to establish a business in the state and the Business Registration Form is used to update or modify existing information and to apply for additional licenses. Both forms must be completed in full in order to be approved by the Debt.
