Hawaii Application For Professional Employer Organization (PEO) Registration is a document required by the Hawaii Department of Labor and Industrial Relations (DLR) for businesses that provide employer services to other businesses in the state. The application is used to register a PEO with the DLR, and must be completed by the PEO’s owner or designated representative. The application includes details about the PEO’s business, its services and its financial and legal status. Once approved, the PEO will receive a professional employer organization registration number indicating it is compliant with Hawaii law. There are two types of Hawaii Application for Professional Employer Organization Registration: Form L&I 736 (for new Pets) and Form L&I 736A (for renewals).
