Delaware Certificate of Service (Fill-In) is a form used by a party to a lawsuit in Delaware to prove that the other party was served with documents in accordance with the rules of civil procedure. There are two types of Delaware Certificate of Service (Fill-In): an Original Certificate of Service, which is filed with the court and a copy which is served with the documents. The form must include the date, name and address of the individual receiving service, type of documents served, name and address of the person who served the documents, method of service, and a signature of the person who served the documents. The form must be signed by the person who served the documents and must be notarized.
